ORDER POLICIES
- Only authorized SupraLife Order Forms will be accepted. Orders mailed or
faxed on unauthorized forms may not be processed.
- All orders must be prepaid with cash, check, Morley order, approved major
credit cards or with commissions (Distributors only).
- The minimum shipping fee is $6.00. For all standard orders of $100
or more, a 6% shipping, fee applies. Shipments outside the continental U.S.
and express shipments will be- -charged actual shipping charges.
- All SupraLife products are sold on a cash or "good funds" basis. Personal checks
may be accepted upon verification of the availability of funds.
- On credit card orders, the credit card must be accepted or the order will not be
processed.
- Credit card orders must have signature of cardholder to be valid.
- Each order must specify only one shipping address.
- Telephone orders will only be accepted if payment is made with an approved
credit card and sufficient 'Credit is available at the time of ordering.
RETURN POLICIES
SupraLife offers a full 30 day money back guarantee, and a 90 day product
exchange policy. If you are dissatisfied with the product you have purchased from
SupraLife, you may return it to SupraLife per the following policies:
• A Return Authorization Form must be returned with the product.
Call SupraLife's Customer Service Department at 1-800-359-3245 and ask
for a Return Authorization Form to be sent to you.
• A Return Authorization Form must be requested within 30 days after
receipt of your order.
• Product must be received by the SupraLife Warehouse within 30 days of issuance
of the Return Authorization.
• Please have the Return Authorization Number prominently displayed on the
outside of the box and include the Form inside the package you are returning.
• All bottles and unused portion of product within such bottles for which you are
requesting a refund must be returned in order for a refund to be given
• The shipping and handling fee is not refundable on returns.
